Permission to use student photos, videos, and recordings. Covers both previously captured material and future captures, channel by channel, with the right to withdraw at any time.

The short version
In plain language
- This form gives Siran Music Academy permission to share photos, videos, and recordings of your child's lessons, recitals, and achievements.
- You are in control. Tick only the places you are comfortable with, for example the website or Instagram, and leave the rest unchecked. You can also choose to share nothing at all.
- It covers both older material already in our archive and new material taken while your child studies with us.
- You can change your mind or withdraw at any time. We remove the material from our own channels within 30 days.
- Signing is completely optional. It never affects your child's lessons, exams, or opportunities.

1. About this form
Why we are asking
Siran Music Academy (the "Academy") is a private music studio operated by Siranoush Akhgarandouz in North Vancouver, BC. As part of teaching and celebrating student progress, we sometimes take photos, record videos, and capture audio of students during lessons, recitals, festivals, exams, and other Academy activities.
Some of this material has been captured over the past several years and is stored in our archive without formal written consent. We are asking now for your clear, informed permission so we can use this material appropriately, in compliance with British Columbia's Personal Information Protection Act (PIPA).
This form asks for your consent for both (a) material we have already collected, and (b) material we may collect in the future while the student is enrolled. You can say yes to some things and no to others, and you can change your mind later. Refusing or withdrawing consent will never affect lesson quality, performance opportunities, or exam registration.
2. Material already captured (retroactive)
For photos, videos, and recordings already in our archive
The Academy holds material going back several years - private lessons, group classes, studio practice, recitals, festivals, examinations, performances, masterclasses, and workshops. This material was captured by Siranoush, by Academy representatives, or submitted by parents or third parties.
By giving retroactive consent on the PDF form, you allow the Academy to keep, use, and publish any and all photos, videos, and audio recordings of the student already captured prior to the date of consent, regardless of when captured - but only on the channels you select in section 4 below.
You do not have to consent to retroactive use. If you decline, the Academy will not publish or further share previously-captured material of the student, and will withdraw it from active publication on its own channels on request.
3. Future captures during enrolment (prospective)
For material captured from today onward
Prospective consent gives the Academy permission to photograph, video record, and audio record the student during Academy-related activities for the duration of enrolment, and to use that material for the purposes and on the channels you select in section 4.
Prospective consent continues until you withdraw it in writing, the student is no longer enrolled, or this form is replaced by a more recent version.
4. Where the Academy may publish the material
Channel-by-channel choices
Your selections on the PDF apply to both previously-captured and future material. Leave unchecked any channel where you do not want the student's image, voice, or name to appear.

- RCM publications - honor roll listings, teacher profiles, similar

Naming - choose one
- Full first and last name - highest recognition value, easier to search by name online
- First name and last initial only - for example "Atrisa B." - balanced choice
- First name only - lower identification risk
- Do not name the student - image or audio only, no name - lowest identification risk
Once material is published online, other users may reshare, screenshot, repost, or save copies. The Academy cannot guarantee removal of material that has been reshared or saved by third parties. Ticking the channel boxes is an acknowledgement of this risk.
5. Your right to withdraw consent
How to withdraw
You may withdraw this consent, in whole or in part, at any time and for any reason. Send a written notice (email is acceptable) to siranoush.akhgarandouz@gmail.com with the subject line "Consent Withdrawal". Identify the student and, if you wish, the specific material or channels you are withdrawing consent for.
Once we receive your withdrawal we will:
- Remove the affected material from the Academy's own controlled channels within 30 days
- Stop using the material for any new purpose
- Retain a copy of your withdrawal request and update our consent records
Withdrawing consent does not affect any use of the material that occurred before we received your request.
6. Students under 19 (BC age of majority)
Parent / guardian signing
In British Columbia the age of majority is 19. For students under 19, a parent or legal guardian must sign on the student's behalf, confirming legal authority to consent.
Mature minor recognition: if the student is 12 or older and is capable of understanding what they are agreeing to, we ask the student to also sign, alongside (not in place of) the parent or guardian. This reflects the Canadian privacy principle of mature minor consent.
When the student turns 19 during enrolment, we will ask the new adult student to sign a fresh consent form in their own name.
7. How long we keep the material
Storage and retention
The Academy maintains a private archive indefinitely unless you request deletion. Archived files are stored on password-protected devices and cloud accounts with reasonable security safeguards in place.
If you withdraw publication consent the material moves to a "do not publish" state but may remain in the private archive. You can request full deletion of the originals at any time.
Under PIPA section 23 you may request access to the personal information the Academy holds about the student. Under section 24 you may request correction of factual errors (a misspelled name, a wrong age). Email siranoush.akhgarandouz@gmail.com for access or correction requests; we will respond within 30 business days.
8. How often we will refresh this consent
Refresh cadence
- Every 3 years while the student remains enrolled, to confirm your preferences have not changed.
- When the student turns 19, so the new adult student can sign in their own name.
- If the Academy adds a significant new publication channel (for example, a new social media platform), we will ask for fresh consent specific to that channel.
- Any time on your request if you would like to update your selections.

Purposes of collection, use, and disclosure
Marketing and promotion of the Academy's services, sharing student achievements with the public and the music education community, educational use including submission of recordings to examination and competition bodies, building a historical archive, and internal teaching review.
Cross-border storage
Some of the Academy's cloud and social platforms (Google, Meta / Instagram / Facebook, YouTube, LinkedIn, TikTok, Telegram, WhatsApp) store data outside Canada and may be subject to the laws of those jurisdictions. By consenting to those channels you acknowledge this.
Complaint mechanism
If you believe the Academy has handled the student's personal information in a way that does not comply with PIPA, contact the Privacy Officer first. If you remain unsatisfied, you may file a complaint with the Office of the Information and Privacy Commissioner for British Columbia (OIPC):
